This information is updated by the health plan at the time of the credentialing/re-credentialing process at least every three years, and when the physician notifies the health plan of any changes. A physician’s affiliation with a hospital does not guarantee the hospital is a participating provider in your plan’s provider network. Please confirm your desired hospital is in-network. Close Accepting New Patients Explained Accepting New Patients indicates if a physician is currently accepting new patients into his/her practice. Physicians self-report if they are accepting new patients or not at the time of initial credentialing and through annual updates thereafter. Limitations to this information may occur if the physician does not inform PacificSource. Requests to update and verify this information are sent to physicians annually. This is the only field that is required and allows us to find the providers within a certain distance of the location you enter. Next, you can choose to narrow your search by the fields available. Can I look up providers by clinic name? Yes, simply begin typing in the clinic name in the Provider or Group Name textbox under the Narrow your search section of the main search screen. How do I find a provider or hospital near my home or work address? Simply enter a street address in the search fields and then limit the driving distance to the radius you are willing to travel. Keep in mind that if you search by City and State, or Zip Code, we will use the center point for the city or zip code as the center of the radius. How do I find a vision provider? Use our online Provider Directory to find a PacificSource network vision provider. In the Specialty Category dropdown, choose Practitioner Categories > Vision Providers to find PacificSource vision providers. If you’re not sure what vision services your plan covers, see your policy, member handbook, or contact Customer Service. What if you don’t have a specialty category for the provider type I want to see? The top level category, called “Specialty Category,” contains the broad field of practice for a provider. To see the specific types of specialties, also choose a “Specialty” underneath it. For example, you’ll find Midwives as a specialty under the Neonatal category. How do I find an Urgent Care Center? Search for “Urgent Care Facilities” as the Specialty Type and then "Urgent Care Providers" as the Specialty. Plans and Networks I don’t know what plan or network I have. How can I find that out? Take out your PacificSource ID Card. You’ll find the name of your plan or network on your card. In order to get the most from your benefits, you’ll want to see a provider that accepts your specific plan or participates in your plan's network. What if my doctor says they are in your network, but I can’t find them?
